af2560222f Harden Stage 5 subscription safety checks 
Fence stale ACK results by generation, make duplicate MarkProcessed harmless after confirmed ACK, and cover forged delivery, mutation, in-flight ACK, initial subscribe, and replacement edge cases.

54e8c73eb7 Test ACK response loss reconnect behavior 
Cover the ambiguous case where the server applies an ACK but the client sees a transient failure, ensuring reconnect follows the server durable cursor instead of local receive state.
